PSC Lost Badge Replacement Form

    To obtain a replacement Member Badge, or both a Member Badge and Smart Card, fill out this form, print it, and mail it with a check for the appropriate amount to:

    The replacement fee for a new Member Badge is $10 and the fee for both a Member Badge and a Smart Cards is $25. Please note, if your Smart Card is replaced, your original Smart Card will be deactivated.

    You must also submit this form electronically, but before doing so, be sure to verify that all of your Member information in our database is up to date. Your replacement badge will be mailed to the address we have on file. To verify, please go to www.PSCmembers.com and log in using your Name, Member No. & birthday.
